In-home and Community Supports

Our In-Home and Community Support Service can support you to develop skills and build independence to live the life you choose.

Our service is all about supporting you to increase your independence and reach your full potential. Supports offered might include assistance for you to be able to take part in social activities, support to attend appointments or simply requiring support at your home.

We can offer supports both in home and in the community, allowing you the flexibility and choice of individual supports you receive, with activities such as personal care, social outings, transport, shopping, doctor’s appointments and cooking.

We provide you with support for everyday activities when you need to leave home. Our staff can provide support with activities such as:

Supporting you whilst your carer is away

Providing you with overnight support

Active awake night care

Supporting you with recreational activities

Support with meal preparation

Basic domestic support

Shopping

Attending community events

Recreational activities such as a concert or gallery

Attending appointments

Accessing other programs (swimming club, sports, or another disability service provider)

We can also accompany you and provide support whilst on holidays or taking a short trip.

Group and Community Participation

Our group and community participation programs can support you to improve skills in:

  • Cooking
  • Budgeting
  • Time Management
  • Social interaction
  • Caring for health and wellbeing
  • Being independent in everyday activities
  • Communication with others

Living independently, We will work with you to understand the skills you want to learn or develop. We focus on building confidence and independence to allow you to truly reach your potential. We offer centre-based groups and can also assist you to attend local community activities, either as part of a group or with one-to-one staff support.

Individual Skills Development


Group support or activities. We have three types of group sessions available.

Centre-based skills development sessions

Workshops which support you to learn and develop skills so that you can participate in, and undertake, activities independently.

Activities designed to allow you to participate in a wide range of enjoyable activities such as cooking, gardening, art and craft, sport or fitness.

Access to community events and activities that focus on supporting you to access community activities such as sporting events.

Skill development sessions that focus on building your confidence and developing your skills to be more independent. Examples include health and personal hygiene, budgeting, independent travel, protective behaviours and domestic skills.

Supported Independent Living

With the help of our Support Coordinators, we can offer assistance in accessing Supported Independent Living services. We can assist you to find a new place to call home and to receive the supports you need in your new home.

Long Term Accommodation
We can work with you to understand what type of housing you need, where you would like to live and who you would like to live with.

This includes working closely with your Support Coordinator to ensure that you have appropriate funding in your plan.

We can provide experienced staff up to 24 hours a day/7 days a week for personal care, healthcare and support to do the things you want to do at home. We aim to find the right home for you, with your own bedroom, fully furnished shared living areas and accessible outdoor spaces and gardens to enjoy.

We understand that the place you want to call home is very personal and we will respect the choices you want to make about the space that call home.

Assisting you to live independently in a great home is something we are passionate about.

Short Term Accommodation - Located at 66 Lyall Street
For short-term breaks, we also provide short to medium term accommodation in our Respite service.

In-home and Community Supports

Our School Leaver Employment Support (SLES) service is a practical, hands-on program, with a genuine focus and commitment to supporting you to become work ready, gain work experience, and explore volunteer opportunities to improve your likelihood of finding a job.

We can assist you to achieve your employment goals. Support is tailored to meet your needs, assisting you to find work experience or volunteer opportunities; learn to socialise within the workplace; feel competent with travelling; and other employment-related skills. These skills build a solid foundation to increase the chances of securing work experience or long-term employment.

Our staff will support you to build skills in:

Communicating with others

Everyday safety

Planning and preparation

Finding suitable and tailored work experience

Domestic and household tasks

Personal care and hygiene

Making decisions

Community engagement

Handling money

Time management

Practical skill development

Therapy Services

Our therapy team is a group of experienced and highly skilled professionals who can work together to provide you with a range of services. We cater to the needs of children, teenagers, and adults. We will offer you personalised services to help you achieve your goals.

You can access Therapy services from us in a range of settings such as at home, in the classroom, the workplace or the community.

Therapists in our service include:

Occupational Therapists - focus on helping people do all the things that they want and need to do in their daily lives. These include social situations, sensory integration, behaviour, household tasks. Occupational therapy aims to promote health and improve quality of life.

Speech Pathologists - assessment and therapy to treat communication disorders, including difficulties with speaking, listening, understanding language, reading, writing, social skills, stuttering and using voice.

Counsellors - Guidance for parents, carers or children in behaviour management, daily routines and strategies needed to support a child or person with disability.

Allied Health Assistants - work closely with our allied health team in helping to maintain your individual therapy goals.

Ability Farm

Ability Network are planning a new venture to be known as Ability Farm.

Situated on acreage, this is a key location from which Ability Network will provide a range of activities for clients. It will also be a community space with multipurpose use buildings, community gardens with farm animals.
